The joint Regional Green Light Committee (r-GLC) and Global Drug Facility mission in Tanzania took place from 8th to 17th November 2021. The mission was primarily to monitor the status of implementation of the programmatic management of drug-resistant TB (PMDT), the laboratory and drug quality and management support system. Other thematic areas included the implementation of active drug safety monitoring and management (aDSM) and infection prevention and control (IPC).

Tanzania is among high TB/HIV burden countries according to WHO classification. Analysis of trends of WHO TB incidence estimates in Tanzania show a steady decline for the last ten years. The country has also seen an increase in TB notifications over the last five years, and the estimated TB incidence rate for new and relapse (all forms) per 100,000 populations has declined by 17% from 306 in 2015 to 253 in 2019, which is in line with the target of 20% decline by 2020 compared to 2015 making the country among the 7 high burden countries on track to attain the 2020 incidence reduction milestones.

In 2016, Tanzania was granted a three years project “Enhancing active case finding and early case detection through contact screening in three endemic districts of Tanzania.” supported by Bangkok Declaration special fund (BDSF). This project aimed to ensure early leprosy case finding and treatment in order to interrupt the chain of transmission especially among 2 household members of index cases. It should also result in a dramatic reduction of the G2D proportion among new cases. The proposal is built in line with the WHO Enhanced Global Strategy for Further Reducing the Disease Burden due to Leprosy (2011‒2015) and the Global Leprosy Strategy 2016‒2020 “Accelerating towards a leprosy-free world”; as well as the National Strategic Plan for Tuberculosis and Leprosy Control, 2015‒2020.

The main objectives of the assessment were to review the diagnostic network, current practices, and algorithms; identify challenges that prevent the overall diagnostic network from performing efficiently and effectively; and propose evidence-based interventions to improve the overall ability of the TB diagnostic network to meet the goals and targets of the NSP.
